OSHA Fall Protection 2025 Reference
Why this reference exists
Falls are the #1 cause of construction + service-trade fatalities in the U.S. (35-40% of construction deaths annually per BLS). OSHA fall protection citations are the most common OSHA violation across all industries. Every roofing, gutter, HVAC, electrical, painting, tree service, + chimney sweep crew works at heights. This is the working summary; serves as both training reference + post-incident defense.
Triggering heights
General industry (29 CFR 1910): 4 ft
- Service-trade work in commercial + residential service
- Most HVAC, electrical, plumbing service trades
Construction (29 CFR 1926): 6 ft
- New construction, major remodel, roofing, framing
Special cases:
- Steel erection (1926 Subpart R): 15 ft minimum (with exceptions)
- Scaffolds (1926 Subpart L): 10 ft
- Excavation/trench: at any depth approaching cave-in risk
For service-trade crews: 4 ft is the working trigger. Above this, fall protection required.
Acceptable fall protection
Three methods in priority order (per OSHA hierarchy of controls):
1. Engineering controls (eliminate the hazard):
- Don't climb if work can be done from below
- Pole tools, extending equipment, robots, drones
2. Passive systems:
- Guardrails (42" ± 3" with toeboard)
- Personal fall arrest system (PFAS) - harness + lanyard + anchor
- Safety nets (typically construction; less common service)
3. Administrative controls:
- Training, work rules, scheduling
- Last resort, not standalone
In practice: most service trades use PFAS for above-4-ft work.
Personal Fall Arrest System (PFAS)
Three components ("ABCs"):
A - Anchor:
- Must support 5,000 lbs per person
- OR be designed by a Qualified Person to maintain a safety factor of 2
- Acceptable anchors: certified roof anchors, structural members (verified), tie-off points designed-and-marked
- NOT acceptable: HVAC duct, plumbing pipe, exhaust vent, sheet-metal stud
B - Body harness:
- Full-body harness only (NOT body belt - body belts banned for fall arrest in construction since 1998)
- Properly fitted; D-ring centered on back between shoulder blades
- Inspected before each use
C - Connecting device:
- Lanyard: typically 6 ft maximum
- Self-Retracting Lifeline (SRL): preferred - limits free fall to under 2 ft
- Shock absorber typically incorporated
- Inspected before each use
Fall clearance calculation (matters when anchor is at foot level):
- Free fall distance + deceleration distance + harness stretch + safety margin
- Typical 6-ft lanyard: needs 18.5 ft of clear fall space
- SRL: needs about 4-6 ft of clear fall space
Rescue plan
OSHA requires a written rescue plan. Suspension trauma can be fatal within 30 minutes; rescue must start in minutes.
Plan elements:
- Designated rescue person on-site (trained)
- Equipment: mechanical advantage system (Z-rig, 3:1, 4:1)
- AED + trauma kit
- 911 backup (not primary)
- Annual or quarterly rescue drills
Service-trade crews often skip rescue plans + rely on 911. This is a citation. Real-world fatalities happen during the 20+ minutes waiting for fire department.
Ladder safety
Most service-trade falls happen from ladders, not roofs.
OSHA ladder rules (1910.23, 1926.1053):
- Extension ladder: 4:1 angle (1 ft out for every 4 ft up). 75 degrees.
- 3 ft above eave for transitioning to roof
- 3-point contact climbing
- Don't carry tools by hand - bucket lift or harness-mounted
- Inspect rails + rungs before use
- Don't stand on top 3 rungs
- Tie off the top to prevent kick-out
- Keep belt buckle between the rails
Common ladder violations + how to avoid:
- Wrong angle (too steep, too shallow)
- No top-tie-off
- Setup on uneven ground (use levelers OR move)
- Setup on wet/icy surface
- Setup near power lines (10 ft minimum)
Roof work specifics
Residential roof work (slope-dependent):
- 4:12 slope or less: warning lines + monitor + PFAS option
- 4:12-6:12 slope: PFAS required
- 6:12+ slope: PFAS required + retractable lifeline preferred
- Wet, icy, or steep: stop work
Anchor points on residential roofs:
- Roof anchor (Werner, Guardian, etc.) attached to ridge or structural member
- Anchor placement matters - must be far enough above work area to allow swing-fall protection
- Single use vs reusable; reusable anchor stays installed for next visit
Skylight + chimney work: anchor + PFAS + secondary safety + ground crew comms.
Service-trade exceptions + nuances
HVAC service on equipment platforms:
- If platform has guardrail, no PFAS needed
- If not, PFAS required at 4 ft
Electrical service at panels mounted on outside walls high up:
- Bucket truck preferred
- Ladder + PFAS acceptable per OSHA standard
Painter on ladder for trim:
- 6 ft+ ladder height: PFAS strictly per OSHA (often not enforced for short ladder spot-work in practice, but the regulation is clear)
- Most painter fatalities happen from extension-ladder fall during reach-too-far
Gutter cleaning:
- Single-story: usually <6 ft eave; ladder safety only
- Two-story: PFAS required at most jobs
Tree service climber:
- ANSI Z133 governs (two-rope system)
- Aerial lift workers in basket: tied to boom
Training requirements
Per OSHA + ANSI:
- Initial fall protection training before working at height
- Topic coverage: hazards, system selection, PFAS use, anchor selection, rescue, inspection
- Retraining when conditions change (new equipment, new site) OR annually as best practice
- Documented (signed roster + curriculum)
Field-team friendly training:
- 1-2 hour annual refresher
- 4-8 hour full course for new hires
- OSHA 10 + OSHA 30 cards include fall protection content
- Some manufacturers (Capital Safety, Werner) offer free training
Citation + penalty math
OSHA fall protection citations:
- Fatality: criminal referral possible + $$$+
Insurance impact: Workers Comp Experience Mod Rate climbs after fall incidents; can double premium for 3 years.
Lost-work-day cost: average fall injury costs employer a substantial five-figure sum in direct costs + lost productivity 200-400 hours.
Common pitfalls
- No anchor selection check: tied off to a vent pipe; pipe pulls out in fall
- Lanyard too long for height: customer's hard hit on the ground 18 ft below the roof
- No fall clearance: PFAS used but fall zone shorter than required clearance = hit ground anyway
- No rescue plan: suspended worker dies from suspension trauma
- Old harness: webbing UV-degraded; fails on activation
- Single-rope climbing on a tree: not OSHA-compliant (ANSI Z133)
- Working alone at height: no rescue available; OSHA citation
- Heat / cold / fatigue: increases fall risk dramatically
Safety culture
Best practice: pre-job briefing for every job > 6 ft.
