Two Faults, One Dangerous: Decision Tree

Why this matters

Sometimes a system has two problems at once, and one of them can hurt someone while the other just annoys the customer. The trap is tunnel vision: you fixate on the loud, obvious nuisance fault and walk right past the quiet, dangerous one. Gas, electrical, pressure, carbon monoxide, and structural hazards do not announce themselves the way a noisy fan or a weak flow does. The discipline is to scan for the dangerous fault first, every time, before you let the annoying one pull your attention.

Safety first: address the dangerous fault before anything else

Lead with the hazard, not the complaint. The moment you suspect any safety-related fault, stop normal diagnosis and deal with it first.

  • If you find or suspect a gas leak, a combustion or carbon-monoxide problem, exposed or energized conductors, a failed pressure-relief path, or anything that could injure a person, that fault takes priority over the customer's actual complaint.
  • The conservative call: make the dangerous condition safe before you do anything about the nuisance. Shut off, isolate, lock out, ventilate, or red-tag as the hazard demands. Do not energize or restart to "test the other problem" while a hazard is live.
  • If you cannot make it safe with the tools and authority you have, shut the system down, inform the customer plainly, and bring in the right resource. Walking away from a live hazard to fix a rattle is never the right order.

Only after the dangerous fault is contained do you continue diagnosing.

Why two-fault calls fool people

The complaint that got you dispatched is usually the one the customer can feel: weak flow, a noise, a unit that will not run. That is the nuisance fault. The dangerous fault is often silent and may even be unrelated to the complaint; it was just sitting there. Because you came for the noise, your brain wants to solve the noise. Deliberately widen the scan before you narrow it.

Start here: scan for hazards before you scan for the complaint

Walk a fast hazard check on arrival, independent of what you were called for.

  • If the system burns fuel: smell for gas, look at the flame and venting, and consider carbon monoxide. A combustion fault can coexist with the unrelated thing you were called about.
  • If the system is electrical: look for heat damage, scorching, exposed conductors, water near energized parts, or a defeated ground.
  • If the system holds pressure: confirm the relief path is intact and nothing has been capped, blocked, or jumpered. (See related: The Temporary Safe-Shutdown Method.)
  • If any guard, interlock, or protective device has been bypassed, treat that as a hazard in its own right.

Anything found here jumps the queue ahead of the complaint.

Branch 1: the hazard is contained, now triage the rest

With the dangerous fault made safe, you can work the nuisance fault.

  • If the two faults are unrelated, fix the hazard properly (or arrange for it), then address the complaint as a separate item. Document both.
  • If they are related (one fault caused or hid the other), understand the chain before you restore power. Clearing the nuisance must not re-expose the hazard.
  • If fixing the nuisance would require energizing or pressurizing the system while the hazard is only temporarily contained, do not. Resolve the hazard fully first.

Branch 2: you can only fix one today

Reality intrudes: parts, access, or authority may stop you from fixing everything on one visit.

  • Always leave the dangerous fault safe, even if it stays unrepaired. A contained hazard plus an unfixed nuisance is acceptable. A live hazard plus a fixed nuisance is not.
  • If the only way to keep the customer in service would leave the hazard live, the conservative call is to leave it shut down and explain why. Convenience never outranks an injury risk.
  • Document what you found, what you made safe, what remains, and what must happen before the system runs again.

The discipline

When a job has two faults, assume one might be dangerous and look for it on purpose before the loud one captures you. Make the hazard safe first, fix the nuisance second, and never restore a system with a known live hazard just to satisfy the original complaint. The right order is fixed by the consequences: the thing that can hurt someone always goes first.
