Ceiling Stain - Active vs Historical Decision Tree
Why this matters
Half of "I have a ceiling stain" calls are not active leaks - they are historical stains the customer noticed today and assumed are new. The other half are active, sometimes catastrophically so. Routing both as urgent dispatches wastes capacity; routing both as cosmetic kills the customer who actually has a leak. The four buckets - active leak from plumbing above, active leak from roof / envelope above, active leak from HVAC condensate / supply, and inactive historical stain - each have specific field signatures that can be confirmed in under ten minutes with a moisture meter. Catching active early matters: ceiling drywall holds about a pound of water per square foot before it falls, and that often happens on someone's head.
Symptom presentation
Five intake questions: when did the customer first notice (today, this morning, last week, "always been there"), is the stain growing or stable, what is above the stain (bathroom, attic, roof, HVAC equipment), did it rain or snow recently, and any drip / drop activity. Add: is there bubbling / sagging in the drywall, any visible water beading on the surface, any musty smell from the area.
Cross-trade quick checks
- Drywall bubbling, sagging, dripping water: ACTIVE catastrophic. Customer puts a bucket and a poke-hole in the lowest point. Emergency dispatch - source trade depends on what is above.
- Stain matches the footprint of a bathroom above: PLUMBING active or recent. Plumber.
- Stain near or below an attic-mounted air handler: HVAC. Condensate or refrigerant. HVAC.
- Stain near an exterior wall, around a chimney, or under a roof valley: ROOFING. Roofer or general carpenter.
- Stain present, ring is dry and tan / brown, no growth: HISTORICAL likely. Confirm with moisture meter.
- Stain appears only after heavy rain: ROOFING or window flashing.
- Stain appears every time AC runs: HVAC condensate.
- Stain appears when nothing specific runs: SLOW PIPE LEAK or roof issue that is no longer active in current weather.
Isolation tree by source
Active plumbing path. Stain footprint matches a bathroom, kitchen, or laundry above. Diagnostic: shut every fixture above, watch the meter. If it spins, supply leak. If stable, run each fixture upstairs separately - shower, sink, tub fill, toilet flush, washer fill - and watch the stain underneath for any change. Common: toilet wax ring (smell often present), tub overflow gasket, shower pan drain, supply-line drip at angle stop, dishwasher supply, washing-machine valve drip. Wax-ring stains typically yellow and grow slowly; supply leaks grow fast. Cut a small inspection hole at the suspect joist bay - the path of staining on the joist points to the source.
Active HVAC path. Stain near an attic or ceiling-mount air handler, or directly below a supply boot. Diagnostic: pull the air handler service door, inspect the condensate pan for standing water (pan full = drain clogged), check the overflow safety switch (Wet Switch, SS3), check the secondary drain pan if present (water in it = primary system overflowed). The condensate trap clogs with bioslime - shop-vac clears it. Refrigerant supply / return-side leaks rarely drip but a frozen evaporator coil thawing on a service call does drip until pan + drain are cleared. Sweat-staining on a supply boot in summer is supply-air duct insulation failure - cold metal surface in a humid attic condenses on the underside.
Active roof / envelope path. Stain only after rain. Diagnostic: head to the attic with a flashlight during the next storm if possible, or look for fresh staining on rafters and decking. Common: failed pipe boot (rubber collar cracked at the plumbing vent), step flashing failure at a sidewall, chimney chase / cricket failure, ice dam refreezing under shingles, valley underlayment failure. Walking the roof to chase the source is a roofer call. Note: leaks track along rafters, decking seams, and ductwork before showing on the ceiling - the stain location on the ceiling can be 8 feet from the actual roof penetration.
Historical / inactive path. Stable, dry stain. Moisture meter reads under 8% in the surrounding drywall (matches reference reading from a known dry area elsewhere in the house). No growth in customer-reported size, no smell, no bubbling. Cause: a past leak (since fixed), a humidifier overrun event, a steam-shower-without-a-fan habit, a one-time roof event in a past storm. Customer needs reassurance plus cosmetic remediation (stain-blocking primer + paint), not a leak chase. Document moisture meter readings on the work order to defend the call - if it leaks next month the customer will say "I told you it was leaking."
Confirming which trade on site
The first tech confirms active vs historical in 10 minutes with two tools: a pin or pinless moisture meter and a flashlight in the attic or ceiling-cavity access. Step one: meter the stain center and the surrounding drywall; under 8% with no rise after fixture use upstairs equals historical. Step two: open the cavity above the stain (small inspection cut or attic access) and visually confirm wet vs dry framing. Step three: if active, identify the trade by following the wet path uphill - water runs downhill on rafters and joists. Step four: document with photos at every step. The moisture meter reading on the work order is the single most defensible piece of evidence for the disposition.
A ceiling stain over a finished living area that reads wet on the moisture meter must be addressed within 24-48 hours per IICRC S500 to avoid Cat 1 to Cat 2 promotion of any water damage involved. Wet drywall holding standing water can fall and injure occupants; cut a controlled drain hole at the lowest point of any active bulge before leaving the site if the source cannot be stopped immediately.
Hand-off protocol when the trade you sent is wrong
Plumber arrives, finds the stain is HVAC condensate: shut HVAC, clear or shop-vac the drain, refer to HVAC trade. Roofer arrives, finds the stain is plumbing: open inspection hole if needed, hand off to plumber. HVAC tech arrives, finds historical stain: document meter readings, leave customer with the option to cosmetic-patch. Every disposition - active or historical - leaves a one-page documentation card: photos, moisture readings, suspected source, and the trade involved. The historical-stain disposition is the highest callback risk because the customer's perception is that "you said it was fine"; the documented readings defend the call when the next ceiling event happens unrelated.
