Clearance Bounce Back Elevated Decision Tree

Why this matters

Post-remediation air sampling that comes back elevated above the outdoor reference (or above the control sample) means the third-party assessor has not signed off and the project is not complete. The reflex is to re-clean and re-test, which sometimes works and sometimes doesn't. The decision tree below separates the recoverable cases (sampling protocol artifact, residual settled dust, missed area) from the deeper problems (incomplete removal, hidden growth, ongoing source) that require a scope redo rather than a re-clean.

Symptom presentation

Project has completed S520 remediation. Containment is still in place. Third-party assessor pulls air samples per the project's clearance protocol. Results come back two days later showing elevated spore counts on one or more inside samples relative to outdoor reference. Specific genera flagged may include the species that drove the original remediation (Stachybotrys, Aspergillus, Penicillium, Cladosporium).

Common patterns: one sample point elevated, others matched outdoor; all sample points elevated, all moderate; one sample point with one elevated genus while others are matched. Each pattern points toward different causes.

Quick checks

Pull the assessor's report and the laboratory data. Verify which genera are elevated, which sample points are elevated, and by how much. The interpretation depends on the magnitude (10x outdoor is a different story from 100x outdoor) and the species.

Identify the elevated sample points. One point elevated near a specific wall or corner suggests localized residual; all points elevated suggests broad re-deposition or ongoing source.

Confirm sampling protocol. Was sample taken inside active containment with negative pressure on, or after containment removal? Was outdoor reference taken on the same day with consistent conditions? Was the sample volume adequate per AIHA recommendations?

Look at the visible state of the remediated area. After remediation, a thorough HEPA cleaning of every surface should leave the area visibly clean. Visible dust at clearance time suggests inadequate post-remediation cleaning.

Isolation tree

Branch A: one sample point elevated, others match outdoor, elevated genera match the original target. Localized residual or missed area. Identify the location of the elevated sample, re-inspect that area, re-clean with HEPA vacuum and damp wipe, re-test.

Branch B: all sample points elevated with moderate magnitude (2x to 10x outdoor). Broad residual settled dust. Aggressive post-remediation cleaning was inadequate. Re-clean all surfaces, all furniture, all hidden ledges and tops; re-HEPA-vacuum air ducts in the area; re-test.

Branch C: all sample points elevated with high magnitude (greater than 10x outdoor). Source still active or significant material not removed. Investigation phase. Look for: incomplete removal of porous material with hidden growth; ongoing moisture source that re-seeded; cross-contamination from outside containment during work.

Branch D: sample contains an elevated genus that was not the original target. Indicates a second source, possibly outside the remediation area. Investigate building for additional sources before re-cleaning the current scope.

Branch E: sampling protocol artifact. Sample taken with containment off, or after work that disturbed dust, or with outdoor reference not matched in conditions. Re-sample under proper protocol.

Confirming diagnosis

Compare lab data points: type of spore, count per cubic meter, sample volume, sample time, outdoor reference data. Each piece of data supports or refutes a specific cause.

Visual re-inspection of the work area. Walk every surface, every joist bay, every cavity. Look for material missed in the original removal scope, residual dust on ledges, traces of fungal staining on stud edges.

HVAC inspection. If the building HVAC was not isolated during remediation, spores have been distributed through the system. The supply registers and the coil are now potential re-sources.

Source check. The original moisture source must be confirmed repaired. A leak that resumed after remediation re-creates the conditions for amplification.

Remediation

Branch A path: localized re-clean. Re-set targeted containment around the elevated sample area. HEPA vacuum and damp wipe all surfaces. Re-test.

Branch B path: broad re-clean. Maintain or re-set containment over the full area. HEPA vacuum every surface, every horizontal ledge, top of doors, top of windows, top of cabinets. Damp wipe all surfaces. Inspect and clean HVAC supply registers and the coil if exposure is possible. Re-test.

Branch C path: scope redo. Stop the re-clean reflex. Investigate for missed material, hidden growth, ongoing source. Pull back drywall or finish where suspect; inspect cavities; assess HVAC system; confirm source repair. Expand scope as needed; re-remediate with proper containment and verify; then re-test.

Branch D path: source expansion. The second elevated genus suggests a second source. Investigate building for additional growth before re-cleaning the original scope.

Branch E path: re-sample under proper protocol. Containment in place with negative pressure on, outdoor reference matched, sampling volume adequate.

In all branches, the customer and the carrier need to be communicated to. A clearance failure is not a normal step; it is a finding that requires explanation and a plan. Honest communication is the difference between a recoverable project and a litigation case.

Repeated clearance failures (more than two) on the same project suggest a fundamental scope problem (incomplete removal, hidden source, ongoing moisture). Continuing to re-clean without addressing the underlying cause is not standard practice and may be evidence of negligence in a future dispute. Stop and investigate after the second failure.
