Why this matters
A cylinder that turns without opening the door is a specific failure mode that tells you the key and the cylinder are both fine - the problem is in the mechanical handoff between the cylinder and the latch retraction mechanism. Knowing the three possible points of failure before you open the door saves time and prevents misdiagnosis. This decision tree identifies whether the failure is in the cam, the tailpiece, or the latch case itself, and maps each to the correct repair.
Step 1: Characterize the spin
With the key inserted and turned (or the thumb turn rotated), pay attention to resistance:

If you cannot insert a key (lockout situation), proceed to Branch A or B based on age and visible wear of the hardware. If you can insert the key, the zero-resistance vs. resistance test is the primary branch selector.
Branch A: Broken or missing cam
What it is: The cam is a small D-shaped, butterfly-shaped, or lever-shaped metal piece attached to the rear face of the cylinder. When the key rotates the cylinder plug, the cam rotates with it and contacts a driver or actuator inside the latch case that retracts the latch bolt.
Why it fails: Cams are typically made of zinc alloy or thin steel. They crack from:
- Years of normal wear, especially on high-use doors
- Forcing the key when the latch is binding (torque overload)
- Incorrect cam installed for the latch case depth
Diagnosis confirmation: Remove the cylinder from the door (unscrew the setscrew or remove the mounting screws, pull the cylinder out). Inspect the rear face. A functioning cylinder will have a clearly visible rotating cam piece. A failed one will have a stub, a crack line, or nothing at all.
Repair: Most cylinders can be re-cammed using a replacement cam from a hardware parts kit. Match the cam profile to the latch case manufacturer - cams are not universal. If the cylinder body is damaged or the cam post is sheared, replace the full cylinder. Re-key the replacement if needed to match the customer's existing key.
Branch B: Broken or disconnected tailpiece
What it is: Some cylindrical locks - especially older knob sets and lever sets - transmit rotation through a flat metal tailpiece (also called a connecting bar or drive bar) that extends from the rear of the cylinder plug through the lock chassis to engage the retraction mechanism directly.
Why it fails: Tailpieces are thinner than cam posts and more prone to snapping at the junction with the cylinder plug. Failure modes:
- Snap fracture from forcing or torque overload
- Corrosion-induced fatigue on exterior doors
- Tailpiece seated incorrectly after a previous repair attempt and pulled free over time
Diagnosis confirmation: Remove the cylinder. Look for the tailpiece extending from the rear of the plug. If it is absent or visibly snapped, this is the failure. If the tailpiece is present but not engaging, check whether it is the correct length for the latch case being used - a too-short tailpiece can turn freely without reaching the retraction driver.
Repair: Replace the tailpiece (sold separately in many hardware assortments) or replace the full cylinder if the plug is damaged at the tailpiece seat. Verify the new tailpiece length against the latch case specification.
Branch C: Latch case linkage failure
What it is: The cam and tailpiece are intact and driving correctly, but the internal latch case mechanism is not responding. The latch bolt does not retract when the actuator is driven.
Why it fails:
- Broken plastic or metal actuator inside the latch case
- Disconnected spring-and-lever linkage from a previous attempt to disassemble the latch
- Corrosion or debris jamming the latch bolt in the extended position
Diagnosis confirmation: With the cylinder removed, insert a flathead screwdriver into the cam slot (if present) and rotate it. If the latch retracts, the cam or tailpiece was the problem. If rotating the driver slot directly does not retract the latch, the latch case itself is failed.
Repair: Replace the latch case. Cylindrical latch cases are inexpensive and standardized. Match the backset (typically 2-3/8 inch or 2-3/4 inch) and confirm the door thickness is within the latch case range.
Opening the door when the cylinder will not engage
When the customer is locked out and the cylinder is non-functional, the options in order of preferred approach:
Latch shimming: If the door does not have a deadlatch plunger engaged, a latch shim tool inserted between the door and frame can depress the latch bevel and open the door without damage. Works only on unlatched (spring-latch) doors with enough frame gap.
Cylinder removal through the mounting screws: If the mounting screws are accessible from the outside (some knob sets have exposed screws), remove the cylinder to gain direct access to the latch cam slot.
Destructive entry - drill: Drill the cam or shear line to allow the plug to rotate freely with a screwdriver. This destroys the cylinder and requires full replacement.
Drilling is a last resort and must never be the first approach on a fire-rated door or labeled fire assembly. Fire-rated door hardware must be replaced with listed fire-rated hardware of equal or greater rating after any destructive entry. Document the pre-entry condition and hardware labels before drilling, and inform the customer that the replacement hardware must meet the original fire rating.
